Certify
Accepting forms of documentation include:
- A medical summary or visit note discussing your medical condition that includes current diagnosis, medication list, and/or any diagnostic testing done that supports your diagnosis such as X-rays, CT scans, MRIs, blood work, and EMG.
- We also accept documentation from your surgeon, chiropractor, or pain management provider.

Register for your Marijuana Card
- After you are certified by Adult Health DNP, you may then register with the NYS Department of Health. https://my.ny.gov/
- You will be given instructions on how to register with the NYS Department of Health along with your medical cannabis certification at the time of your first visit.
- In order to register you must be a resident of NYS.
- After you have registered, you may then print a temporary medical marijuana card which will be active for 30 days. This temporary card will allow you to go to a certified NYS medical marijuana dispensary until your actual marijuana card is mailed to you. Your actual marijuana card will be mailed to your home in approximately 2 to 3 weeks.

Receive
Congratulations! You should now be officially registered with the program. To purchase medical cannabis, you will need to go to a designated NYS medical marijuana dispensary. In order for you to be allowed into the dispensary you will need to have the following or you will not be allowed to enter the dispensary:
- Temporary marijuana card or actual marijuana card
- Certification/recommendation
- Valid government-issued ID: state ID, driver’s license or passport
- DO NOT bring: Non-registered patients, including children or other companions. Non-registered visitors may not enter the dispensary, unless they are an assigned caregiver.
- Most dispensaries accept cash, credit or debit.
- No appointment is necessary. All dispensaries take walk-ins. If you are a new patients, we would like you to come in an hour before closing.
- When you pick up your medical cannabis, you will meet with a licensed pharmacist who will discuss your health history with you, including your other medical conditions and prescribed medications. During your consultation, the pharmacist will work with you to recommend a type of medicine and dose tailored to your specific needs. All program pharmacists have studied the clinical data, dosage guidelines, best practices and safety protocols of other countries and states with longstanding medical cannabis programs. You are encouraged to ask questions both during and after your consultation with the pharmacist.
