MEDICAL MARIJUANA FAQS

1) What do I need to do each year to stay enrolled in the Program?

Your status as an enrolled patient in the Program expires one year from the day indicated on your certification. To continue to participate in the Program every 12 months, your certifying provider must extend your certification. You will need to show up for all of your follow up appointments. If there is a missed appointment, there is a possibility your certification will be cancelled.

2) Do I have to schedule an appointment to pick up my medical cannabis?

No appointment is necessary. All NYS dispensaries take in walk-ins.

3) What types of medical cannabis are available in New York State?

In New York, all medical cannabis is smoke free, and is available in pill, liquid, oil, flower, vapor, topical formulations, edibles and more.

PILL : Medicine in a pill, capsule or tablet form allows you to measure a highly precise amount.

LIQUID : After distilling medical cannabis into a liquid tincture/oil, the medicine may be dabbed in droplets in your cheeks or underneath your tongue. It is then absorbed into your system through the mucous membranes inside your mouth.

Vapor: When medical cannabis is extracted into a concentrated oil and heated slowly, it transforms into an inhalable vapor.

TOPICAL : These are preparations meant to be applied externally to the skin or mucous membranes. Typically, they are in the form of creams, gels, lotions, ointments, or patches.

4) Does Adult Health: DNP accept insurance?

Adult Health: DNP does not accept any insurance. We accept cash and credit or debit card. Veterans will receive a discount.

5) What is the process of getting a recommendation/certification for medical marijuana?

When you arrive at your appointment you will be asked to fill out a medical history form and other documents and disclosures. You must provide a valid state-issued ID or driver’s license prior to your evaluation. You will then meet with our doctor, who will evaluate you for the medical cannabis treatment. If you qualify you will receive your certification that same day.

6) What medical documentation should I bring?

It is important for you to bring any medical documentation (from previous doctor visits, ER visits, chronic pain doctor, etc.) that supports your qualifying condition for medical marijuana treatment. This includes medical records, prescriptions, physician’s letter/referral, chiropractor letter/referral, surgical reports, and X-ray, CT, MRI, and EMG or any other document that supports your qualifying condition.

All medical records can be faxed to 716-331-3008.
